该剧情改编自余华的同名小说。富家子弟福贵（葛优）沉迷赌博，妻子家珍（巩俐）多次劝说无果后，带着女儿凤霞离开了他。当晚，福贵输光全部家产，气死了父亲，只能靠变卖母亲的首饰租了间破屋维持生计。一年后，家珍拉着凤霞，怀里抱着刚出生的儿子有庆回到家中。福贵痛改前非，开始以演皮影戏安分过日子。 然而，好景不长。内战时期，福贵被国民党抓去当劳工，历经波折终于回到家乡与家人团聚。此后，凤霞因病成了哑巴。在随后的大跃进运动和文化大革命中，福贵虽有一些小幸运，但逆境始终如影随形。

《活着》主要讲述了20世纪40年代，福贵（葛优饰）因嗜赌成性导致家破人亡，妻子家珍（巩俐饰）带着孩子离开，福贵戒赌后家珍回归，一家人在贫困中相依为命。内战期间，福贵和春生（郭涛饰）被国民党充军，后被解放军俘虏，最终释放回家。20世纪50年代，福贵因家境贫寒被定为城镇贫民，逃过一劫，而龙二（倪大红饰）因地主身份被枪决。大跃进时期，福贵的儿子有庆（董飞饰）被区长（即春生）意外撞死。文革中，春生遭迫害，妻子自杀，他将积蓄交给福贵后企图自杀，但被家珍劝住。凤霞（刘天池饰）与二喜（姜武饰）结婚后生下馒头，但因产后大出血去世。影片最后，福贵、家珍、二喜和馒头在凤霞和有庆墓前祭奠，福贵对馒头表达了对未来的希望和憧憬，相信日子会越来越好。
